Important: The information you enter will not be stored. The PDF document will load in a new window or tab (depending on your browser and settings). If you need to change your information, close the tab containing the pdf document and look for your original tab which will contain the form with your information. Change your information and resubmitt the form. Once you close the window with the form (i.e. this page), your information will be lost. Should you have problems creating/printing your labels and seller's sheets, call Susan at 217-595-5052.
If you do not have a seller number, you need to get one. Please register as a seller with Jerry, our Membership Manager or send an email to sellers at champaignfish dot com. If you choose to print your labels and seller's sheets without your seller number you must write your seller's number on all labels and seller's sheets on the lines provided.
If need be, you can download a blank form and hand write all information.
A PDF reader such as Adobe Acrabat Reader is required to view and print all forms and labels.
